Default Requesting medical records?

Quick help please, I was air lifted to work after an accident, I now need my records to prove what injuries where sustained.

Any idea which records I need or both? I can't see it explained anywhere.

Medical record and/or A&E record

Any ideas or should I just request both.

The hospital should inform your doctors by letter of any treatment you've had, it won't be as comprehensive as your doctors notes but my understanding is that it should be recorded there for your doctor to do the follow up appointments.

It wasn't forwarded to doctor as I couldn't remember doctor or surgery, just need a record as it's kicking of at work, so need some proof.
I have the hospital request record form, just not sure if I need
Medical records or A&E records or both. Im assuming just A&E?
