Press 5 to select the bomb then press your "fire" key. You'll then see the bomb in your hands instead of the gun. Now stand in the "bomb zone" and HOLD your fire button down until the progress bar is complete.

If you're a CT, to defuse a bomb, stand next to it and hold down the "use" key. Standard key is "E" although I've changed mine.
